Hattie Larlham is currently seeking a Social Work Associate to provide high quality liaison services between Hattie Larlham and the families and guardians of the persons served by Hattie Larlham.
Provides supportive services including assistance with guardianship, respite services, funeral/burial pre-planning, human rights, religious services, demographic data collection, and other needs as they arise.
Opening: Full-Time Social Work Associate Monday - Friday, 1st Shift, flexible schedule, occasional weekend and evening hours are required.
Based in Mantua, Ohio - 124-bed facility in Portage County Qualifications: An associate degree, preferably in a human services field, from an accredited college; preferred.
Previous work experience in Intellectual/Developmental Disabilities field is preferred.
Must be proficient in Microsoft Office, Office365, PowerPoint, Word, and Excel required.
Must be knowledgeable in or willing to learn the ICF/IID, Medicaid, Social Security, and Licensure rules and regulations that pertain to this position.
ODDP and Behavior Support training/certification and DODD training is required within first three months in this position and must be maintained.
Ability to travel within the state of Ohio required.
Duties: Responsible for assisting the Respite program to secure funding and to manage our licensed capacity.
This includes managing the calendar for respite visits.
Chairs the Human Rights Committee with responsibility to advocate for the dignity and rights of persons served by Hattie Larlham.
Administers trainings for Committee members annually as required by DODD.
Compiles and presents analysis of relevant data annually as required by DODD.
Completes Restrictive Measures Forms annually as required by DODD for any resident who requires restrictions including visitation, medications, mechanical, etc.
Reviews restrictive measures plans and ensures required consent has been obtained and documented.
Assist families/guardians with required guardianship duties including helping families complete and submit the annual guardian's report, expert evaluation, and annual plan.
Assists families/interested parties to apply for guardianship when a person served is in need of a guardian and liaisons with the local Probate Court.
Manages the Client Information database including demographic information, medical diagnoses and functioning information which is required to be kept current.
Updates Client Face Sheets as needed for entry in the database.
Assists with DODD client portal applications – Level of Care, ODDP, Bed Hold days, etc.
Assists in mailing letters and sending emails to parents and guardians as needed to apprise of any updated information relative to ICF services.
Coordinates and assists families/guardians in obtaining and maintaining the appropriate funding for Hattie Larlham residential services and up-to-date Hattie Larlham consent forms.
If possessing a degree in Social Work, the Social Work Associate will participate as a member of the Interdisciplinary Team for a caseload of residents.
Coordinates and oversees family/guardian for end-of-life planning including obtaining/managing irrevocable burial trusts, organizing religious/memorial services, etc.
