Hattie Larlham is looking for a Part-Time Health Services Liaison who wants to be part of a mission based on Comfort, Joy, and Achievement of people with intellectual and developmental disabilities in Mantua, Ohio.
The Health Services Liaison is responsible for scheduling, transporting and attending resident visits with community health providers while providing routine liaison services between Hattie Larlham’s medical/clinical staff and community health providers for the highest quality care.
Must provide accurate and timely updates and filing of resident’s medical records per filing guidelines and HIPAA privacy rules.

Requirements & Qualifications:
High school diploma or GED.
Vocational/technical training beyond high school preferred.
Must maintain current CPR certification.
Must have a valid Ohio driver’s license with satisfactory driving record.
Previous direct care experience preferred, but not required.
Required to have a good working knowledge of office practices and be proficient in Microsoft Office products and other computer programs used by agency, filing procedures, and necessary office equipment.
Ability to travel within the state of Ohio required.
Duties:
Maintains all persons served records (current, overflow, and discharge records) to ensure pertinent information is available and accessible for agency purposes or surveys.
Performs duties which pertains to persons served clinical appointments.
Communicate effectively with employees and leadership team.
