The Nurses Case Manager is responsible for supervision of the West Cleveland territory under the Direction of the Clinical Manager and complete visits in group home settings
Monday, Tuesday, every other Friday and one Saturday/Sunday per month 6 am - 8:45 am and 3:20 pm - 8:40- pm

RN Case Managers oversee the long-term care for their patients by planning, organizing, and directing home care services.
The Registered Nurse Case Manager is experienced in nursing, with emphasis on community health education/experience.
The professional nurse meets the needs of individuals and families within their homes and communities and is responsible for providing quality patient care via the development, implementation & evaluation of individual patient care plans.
The Case Manager is a liaison between the CBNS, patients, and their families or caregivers/staff and follows established professional standards of care, CBNS guideline policy and procedures in the performance of work responsibilities.
Essential Job Functions/Responsibilities:
Patient Care:
Complete assessments of patient and family to determine home care needs.
Provides a complete physical assessment and history of current and previous illness(es) that corresponds with the physician's orders through assessment of patient's needs, condition, environment, consultation with other health team members, at the time of visit and updates plan of care regularly as necessary and documents this care following CBNS procedures.
Includes the patient and the family/caregivers/staff in the planning process.
Initiates appropriate preventive and rehabilitative nursing procedures.
Administers medications and treatments as prescribed by the physician.
Provides health care instructions to the patient as appropriate per assessment and plan.
Responsible for the delivery of competent, quality patient care in the home.
Maintains communication with Clinical Manager, CBNS Staff and all other treatment team members
Provides skilled nursing care visits that conform with CBNS patient care and general medical policies.
Participates in Case Care Conferences, as needed
Maintains a continuing knowledge of competencies related to the Nursing profession by participating in formal education programs, conferences, workshops and professional organizations.
Participates in on-call rotations with other case managers as scheduled by CBNS.
Ensures that arrangements for equipment and other necessary items and services are available.
Instructs, supervises and evaluates LPN care provided when assigned by clinical manager.
Performs OASIS assessments
On-call for assigned territory M-F and participates in company-wide on-call every 7 weeks.
Communication:
Prepares clinical notes and updates the primary physician when necessary and at least every sixty days.
Communicates with the physician regarding the patient’s needs and reports the changes in the patient’s condition, for delegated personnel; obtains/receives physicians’ orders as required.
RN's supervised by the Clinical Manager will report any actions and orders they received for clients under their care.
Communicates with community health related persons to coordinate the care plan.
Position Qualifications:
Registered Nurse currently licensed in the State of Ohio.
Minimum of one (1) year’ experience as a licensed, registered nurse, preferable three (3) years including home care clinical experience.
Preferred experience in a supervisory or quality assurance position in acute and/or home health.
CPR certification
Valid driver’s license
Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
Prolonged or considerable walking or standing.
Able to lift, position or transfer patients.
Able to lift supplies and equipment.
Considerable reaching, stooping, bending, kneeling or crouching.
Visual acuity and hearing to perform required nursing skills.
