*CHN is seeking a RN Case Manager/Intake Coordinator for Stark County.
Travel to Wayne County is possible when on-call.

Case Managers are responsible for completing OASIS assessments at start of care, recertification and completion of supervisory visits.
Skilled visits as caseload requires with reasonable productivity requirements.
Main focus for job position would be Start of Care assessment/recertifications/visits and/or resumption of care visits.
Manages LPN's or RN's that assist with skilled visits that CM manages.
On call rotation every 7-8 weeks, with compensation allotted for on call week.
Would be responsible for some skilled visits and mediation passes.
*Mileage reimbursement, company issued tablet/laptop for charting, holiday pay, dental, vision, life insurance, and sick time offered.
